Proof of Resident (Include Notarized Residency Affidavit if necessary)

Proof of Residence (Include Notarized Residency Affidavit if necessary)

Accepted forms of Resident Status/Proof of Address (must be current and include the physical address):
- Mortgage statement or Lease/rental agreement

Current gas, electric, or water bill (within 30 days)

Current government agency mail such as food stamps, Medicaid, DFACS, or SSI statements (within 30 days)
Residency Affidavit:
- Must be attached to Proof of Residence if it is in a different name.
- Ex. Guardian and student lives with a family member.
- Notorizing is available in the Enrollment Center

To transfer from one Chattooga County School to another Chattooga County School, you must come to the Enrollment Center with Parent ID and current Proof of Address showing the address you have moved to.
Online Enrollment Information:
- Please have all required documents available to submit through online system before beginning.
- After submitting a completed application, you will be contacted by the email provided regarding enrollment status and if approved, assigned a start date.
